Keamari UC supervisory focal person, UCMO/UCIO, vaccinator, social mobilizer, and mapped birth providers
Duration
9 months, full-time (November 2026 – 31 July 2027)
Responsibilities
Purpose
To support the newborn case registration and tracking in the assigned Union Council, ensuring that every notified birth is recorded, shared same-day with the Keamari UC team, and tracked through follow-up, vaccination, registration, and closure.
Key Responsibilities
Receive and record all birth notifications from mapped providers in the UC case list.
Share every valid notification with the Keamari UC supervisory focal person on the same day.
Maintain the UC newborn case list and shared tracker as the single tracking tool for the UC.
Track each case from notification to closure, coordinating with the social mobilizer for household follow-up and the vaccinator for birth-dose vaccination and registration.
Monitor timeliness of notification, follow-up, vaccination, and registration.
Flag delayed or unresolved cases and escalate pending cases to the District Coordinator.
Support the weekly UC review with case summaries and pending-case lists.
Support mapping and orientation of providers, and maintain regular contact with them.
Submit a monthly UC report to the District Coordinator.
Qualifications and Experience
Bachelor's degree in Social Sciences, Nursing, or Public Health.
Related experience in Maternal and Child Health (MCH) and immunization programs.
1–2 years' field experience in community health or immunization; familiarity with the assigned UC preferred.
Basic MS Office skills for maintaining the UC case list, trackers and presentations for monthly performance review meeting
Fluency in Urdu and the local community language(s); willingness to travel daily within the assigned UC.

Performance will be reviewed monthly by the District Coordinator, with a formal review at mid-term and at the close of the contract.
