Description
Kindle Care is seeking a dedicated and organized Care Coordinator to join our team. In this role, you will be responsible for managing and coordinating patient care services to ensure seamless and effective healthcare delivery. You will work closely with patients, families, and healthcare providers to develop care plans, schedule appointments, and facilitate communication across interdisciplinary teams. The ideal candidate possesses strong writing skills and a background in psychology or English, enabling them to effectively advocate for patients and document care processes accurately. This role does not involve managing a team but requires excellent organizational and interpersonal skills to coordinate resources and support patient needs efficiently. As a Care Coordinator at Kindle Care, you play a vital role in connecting patients with the right healthcare services and educational resources, addressing clinical risks, and ensuring patient safety. You will utilize patient management systems and electronic health records to maintain accurate and up-to-date information, facilitating timely referrals and follow-ups. Your ability to conduct risk assessments and provide health education will contribute significantly to improving patient outcomes and enhancing the patient experience within our healthcare network.
Responsibilities
- Coordinate patient scheduling and referral management to ensure timely access to care services.
- Develop and implement individualized care plans in collaboration with patients and healthcare professionals.
- Manage electronic health records to maintain accurate, confidential, and comprehensive patient information.
- Facilitate communication among interdisciplinary teams including physicians, nurses, and social workers to optimize patient care.
- Advocate for patients by addressing their concerns, needs, and preferences throughout the care process.
- Conduct clinical risk assessments to identify potential health issues and develop strategies for mitigation.
- Provide health education to patients and families to promote understanding and adherence to care plans.
- Coordinate healthcare resources and community services to support comprehensive patient care.
- Use patient management systems effectively to track care activities, referrals, and outcomes.
- Monitor and evaluate clinical risk management practices to enhance patient safety and quality of care.
- Ensure compliance with regulatory requirements and organizational policies related to patient care and documentation.
