The Physician Assistant at Revex Square plays a crucial role in supporting supervising physicians in delivering comprehensive patient care. This position requires expertise in diagnosing and managing patient conditions, performing clinical procedures, and providing preventive care under the guidance of a licensed physician. The Physician Assistant collaborates closely with the healthcare team to design and implement individualized treatment plans and manages medication prescribing within the scope of practice. A key aspect of this role is the proficient use of electronic health record systems to document clinical findings, monitor care plans, enter prescription orders, and manage referrals efficiently. In this role, the Physician Assistant actively contributes to patient education and counseling, empowering patients with knowledge to effectively manage their chronic conditions and medication regimens. The role also involves coordinating care across multiple healthcare providers to ensure seamless patient outcomes and compliance with healthcare regulations and organizational policies. Candidates should have a minimum of two years of experience in a clinical setting and demonstrate the ability to support complex care coordination without direct supervisory responsibilities over a team.

Responsibilities

• Assist supervising physicians by coordinating patient scheduling and managing appointment logistics.
• Review and analyze patient medical records and clinical documentation for accurate diagnosis and treatment planning.
• Support the development and execution of individualized patient care plans under physician supervision.
• Continuously monitor remotely transmitted patient health data, identifying and reporting significant clinical findings promptly.
• Conduct follow-up communications with patients to ensure effective care coordination and adherence to remote patient monitoring and chronic care management services.
• Document all patient interactions, clinical assessments, and coordination activities accurately within the electronic medical record system.
• Educate patients regarding chronic disease management, medication compliance, and the correct use of remote monitoring devices.
• Collaborate with physicians and multidisciplinary healthcare professionals to maintain continuity and high quality of patient care.
• Ensure compliance with HIPAA regulations, healthcare policies, and all applicable organizational and legal standards.
• Manage EHR-based care plan monitoring, prescription order entry, and referral management to support timely and effective patient care delivery.
